Missing file history

Today I had a weird thing happen. I checked out a file, made a change, and checked it back in, just like a thousand other times. But there is no history of the change! The change occurred, that is, I can see my changes in the file in the repository, but there's absolutely no history. The other history for that file is still present and correct. There's no history at the folder level either for that change. Never had Vault do that before. Anyone know under what conditions this happens?

Are you using integration with Visual Studio, or do you perform your check outs and check ins from inside the Vault GUI client?

Is the file you checked in sitting in your Pending Changes tab? If so, then the check in may have failed.

Perform a Show Differences on the file from inside Vault, and select the option to diff against the current version in the repository (the second option in the diff options window). Are there any differences?

Just using the regular Vault GUI client.
Nothing in pending changes, I knew to check that in case it had failed
Show differences against the current version in the repository compares successfully, but as I already mentioned, the change itself DID actually happen. All that's wrong is that the history did not update, as if the change never really happened.

Could you go to Help - Technical Support in your Vault GUI client and copy the client and server information in a post? It's just general environment information. Make sure you are logged into the Vault server when you do that.
