Unable to Open Vault Client - File Already Exists Error

When I try to open the Vault client I get an error: <filepath> already exists. The file that is listed was created and put into Vault using Eclipse and the Vault plugin. I read some other posts about possibly having to clear some cache files but I cannot find the files. Any ideas?

Vault client is Version 4.0.4

What operating system is your Eclipse plugin on? If you're on Windows, see this KB article:
