DB Error when checking out files

We recently (within the last week) upgraded from Vault 3.x / Dragnet 1.x to Fortress. All was well until this morning when developers began receiving the following error message when attempting to check out a file from the Fortress Client: "A database error has occured (FailDBInsert)".

Devs can log into Fortress, browse projects, etc, but check outs just don't work.

I've checked the webserver where the webservice is hosted and the SQL Server where the databases are running and all appears well on both.

Everything worked fine up to around 6PM EST last night as far as I can tell.

Ideas?

We'll need to see a copy of the Vault Server log. It's sgvault.log in %windir%\temp\sgvault.

I've emailed the log as you've requested. Yes, we're on Fortress 1.1.1.

We've worked with Jeramie to figure out this issue. His server log was full of the error "System.Data.SqlClient.SqlException: Failed to load msxmlsql.dll." He determined that the problem was on the SQL server. He resolved the problem by reinstalling SP4 for SQL 2000.
