The check out dialog is no longer showing files by folder:

Did you change between VSS and CVS style? That is set in the Vault Tools - Options - Concurrent Development Style. In CVS mode there is a button to choose which way it displays the files, and that is the only way I found I could reproduce what you are seeing.

I tried switching between VSS and CVS styles and the dialog remains the same.

After you changed styles, did you not have the buttons at the top to change to a tree view? Can you post a screenshot? Check this first.
If still no change, then shut down all of your Visual Studio clients. After they are closed, go to your registry at HKCU\Software\SourceGear\Vault<or VaultPro>\Client\Settings. Look for the registry string for "VsipCheckoutDialog." Rename that string to something else, such as VsipCheckoutDialogOLD, and then restart VS and test it. Any change?
[If you are running Visual Studio as administrator, then you need to change the keys for administrator.]

Thanks for the reply. Changing from VSS to CVS and back had no effect. Screenshot below.
But, renaming the registry key did work! I still do not have the toolbar but the directory tree structure has returned

How it displays in VSS mode is what you were wanting, right?
I can't make the icons on check out I thought I saw come up again, so I may have made an error when I tried it. No one else here can either, so my mistake.
