Auto-checkout inconsistency

We have just started using Vault Standard this week.
We develop in Visual Studio 2010, and were previously using Source Safe.
We have selected Sourcegear Vault Standard Classic client as the plug-in in VS.
Require Exclusive Locks is selected on the server.
We have noticed inconsistencies with the checking out of files when editing in VS2010.

The behavior we expect is: when we make a change to a checked-in file, the file is automatically checked-out.
This happens sometimes, but not all times.
Sometime it just goes ahead and lets us make the change without checking it out - which dangerous with our work practices, as we don't know which files we have edited.

How can get our expected behavior consistently?

In Visual Studio go to Tools - Options - Source Control - Environment. What do you have set for One Save and On Edit?

Is the option to Allow Checked In Items to be Edited checked? If so, uncheck that option.

Are any of the items that you wish to edit prior to actually editing them already listed in the Vault Pending Checkins as an add, delete, or other action?

On Save: Check out automatically
On Edit: Check out automatically

Allow checked-in items to be edited is not checked.

Next time I encounter the issue I will let you know about the Pending Checkins status.

It seems to be behaving itself for now.

All the Options mentioned above were set the same when the issue was occurring (the default, I believe).

When creating a new branch to try and induce the unexpected behaviour and answer your question, we encountered another issue, but rather than tack it on to this post, I will detail it in a new topic.
