Share ops fail "A database error has occured (FailDBReader)

I am receiving
Item XXX caused the transaction to fail: A database error has occured (FailDBReader)
When attempting to share a particular folder into another particular folder. This also occurs if I share it somewhere else first and then attempt to move. It seems to be any destination folder that is two or more levels down under another particular folder. I can share other folders in ok. There did used to be folders with this name before which I have deleted to share in a new branch with the same name.

From looking at the forums I can see others have had similar problems before and they have normally had to supply their sgvault.log file and support has then needed to provide a sql script to fix the problem. The error in the sgvault log file begins as follows:
System.Data.SqlClient.SqlException: Violation of PRIMARY KEY constraint 'PK__#44CA377__956BCC4746B27FE2'. Cannot insert duplicate key in object 'dbo.@tblfsobjectshares'.
Are you able to provide a script to fix this issue or do you need the full sgvault.log file?

What version of Vault are you using?

We would like to see the Vault Server log to make sure nothing else is going on.
It's called sgvault.log and is in %windir%\temp\sgvault on the server machine.
