Could you go to your IIS Manager, expand the website that Vault was installed under, right-click the vaultservice virtual directory and select properties? Next, go to the Directory Security tab and click the topmost Edit button. Is Anonymous Access enabled? What user is entered? If you don't have it checked, add a check next to Windows authentication. You will be able to remove that check later once we are finished here.
Once Windows authentication is checked and saved, try pulling up http://<your vault server>/vaultservice. Does it require you to log in?

Go to where the vaultservice directory is on disk. Right-click it and select properties. On the security tab, I'd like you to add the Everyone group and give it full control. This is just a temporary change to help narrow down the issue. Normally, when the IUSR account is having problems, if a person selects Windows authentication as an option when IUSR can't log in, then a login window will appear. Since that doesn't happen either, I'm thinking that we need to be looking elsewhere for cause of the permissions problem.
