On occasion when users try to log in they get an error message as show in attached file (ERROR MESSAGE 1)
There also appear to be errors logged on the web server around the same time - they are included in the attachment also.

No one else has ever seen this happen (where the event viewer shows the aspnet process segfaulted. Do you have other ASP.Net apps running on the same server?

This is the only web app running on the machine.
Actually, this machine is dedicated to the Vault - the only apps running are SQL Server 2K and the Vault.

Does the Vault server log give any clues? Try setting the logging level to Debug. and see if you can find out what operation was being performed before the restart.

Log level -- use the Vault admin tool.
Log file location -- usuall at c:\windows\temp\sgvault\sgvault.log -- use the admin tool to verify location.
