We have just upgraded to version 4.1 of Vault Client.
One user cannot see licences.licx file contained within one of our projects. All other users can see this file, even using the same client machine.
If the user logs in via the web interface, this file is visible.
Before the upgrade, this user was able to see this file contained in the repository.
If the user tries to re-add this file to the project, the client returns the error :
Item $/wwwroot/***********/licenses.licx caused the transaction to fail: An item with the same name or object id already exists in the selected folder.

The first thing I'd suggest is clearing the user's client-side cache file. There may be information out of sync with the server. Close any open clients (GUI, Visual Studio) first.

After you've cleared the cache, you may also want to restart IIS (use iisreset command from the command line) to clear the server cache before he opens his clients again.
Let us know if this doesn't resolve the problem.
