Upgrade from 3.0.5.2833 to 4.1.0.16216 failed

This user called and we suggested he first upgrade to 3.1.9.

As Linda advised, I first upgraded to 3.1.9, then to 3.5.3, then to 4.1 and everything worked correctly. Thanks for the help, Linda.
