Possible Corrupt Data

I have a newly created document in vault. If I check it out, I see two items in the Pending Change Set pane instead of one, and if I try to check it back in (regardless of whether I make changes) I see two lines there also. If I click OK it produces an error trying to check it. The error is:

A database error has occured (FailDBFault)

If I clear one of the check boxes when I check the file in, it works and the file is updated. I have also renamed the document and the same doc with a new name works fine, so I have a couple of ways to work around this, but I am concerned that data may be getting corrupted in vault or other problems may occur that I am not noticing.

I ran a dbcc-checkdb from SQL server and SQL did not report any problems.

Any suggestions?

Which version of Vault are you using?
Are you using it integrated with Visual Studio?

Vault version is 4.0.4. The developers use Visual Studio but I don't think it's neccessarily 'integrated'. At least I didn't do anything to integrate them and I am the System Admin. The file I am having trouble with is a simple word document.

The first thing you should try is upgrading your client and server to Vault 4.0.6. It's a free upgrade. I think your issue is related to one we had fixed in the last release.
