Error upgrading to 4.0.6: 'sgvault' is already open ...

I am having immense trouble upgrading a Vault server from 4.0.1 to 4.0.6

Here's what happens ...

OK
Configuring your new Vault Installation
Checking for IIS Version...Requesting Vault Admin user password...OK
Connecting to the SQL Server...OK
Verifying the SQL Server requirements...OK
Checking for an existing databases...Found.
Asking for database's fate...Keep existing.
Upgrading the existing source control database...
This may take a long time. Do NOT stop this process!
Checking the installed database version...OK
Grant database access to NT AUTHORITY\NETWORK SERVICE...OK
Upgrading SourceGear master database schema...OK
Upgrading SourceGear source control database schema...OK
Upgrading SourceGear master stored procedures...OK
Upgrading SourceGear source control stored procedures...Database 'sgvault' is already open and can only have one user at a time.
Vault Setup is exiting due to a failure or cancellation. Error Code = -1280

The same result occurs regardless of whether I use Windows or SQL auth (sa or other).

I found ONE other post related to this, but it was not clear how that was resovled - and anyway, it was for v2.x to v3.5 upgrade.

Why is this so difficult?

Alternatively, ... can database be upgraded manually?

I CAN get 4.1.6 installed fresh, but that leaves the none-too-small matter of upgrading the database.

Can that be done manually? i.e. is there an database upgrade script, or perhaps a command line switch for the installer?

By running the installer the 2nd time and choosing Repair it successfully upgraded the database.

I still think upgrades could be made much easier.
