Transaction failed:database error has occured (FailDBReader)

Hi,
I have v2.0.6 of Vault server product. The Vault and DB are on separate servers. When renaming a folder in the repository we get error: A database error has occured (FailDBReader).
The Vaut log file registers the error:
>>Violation of PRIMARY KEY constraint 'PK__@@tblfsobjectsha__6E1F34F2'. Cannot insert duplicate key in object '#6D2B10B9'..
The statement has been terminated.<<
In the sgvault database I can not see table tblfsobjectsha.
Does anyone have any suggestions to help me?
