Exception

When trying to add a new folder to Vault, I am getting the following message:

An exception was encountered during the transaction. Exception: Exception of type System.Exception was thrown. at VaultClientOperationsLib.ClientInstance.Commit(ChangeSetItemColl givenItems

Any ideas?
