Unable to Check In Files

I have a user trying to check in a file he had checked out and modified.

The file seems to have been renamed, although it does not show in the history. On checkin attempt, he gets a window showing 2 files - current file name and old file name, with checkboxes beside each. If he ignores the window and does not check anything, nothing is checked in. If he checks on one, then the file is checked in.

Why would Vault still try to check in a file into its old name, instead of automatically picking up the new name? Is there a way around this?
