Failure on Commit Only

Last week our server ran out of disk space and a sql server backup operation failed. Vault was not functional at all until I cleared disk space and set the database out of single user mode. Now, all vault operations work (login, get latest, etc) except Commit Changes. Commit fails, message "A database error has occurred (FailDBInsert)". All other operations work properly. Server and clients have been upgraded to 3.1.1 from 3.0.7 and problem still occurs.
