A database error has occured (FailDBReader)

Users are getting this message when opening the repository ( other repositories are ok ), the repository will open after clearing the message. Same message will be displayed when the user tries to check out a file, but the check out will not occur.

We are using Vault v3.0.2

I was able to get a peek inside jschuit's database, and remove a duplicate row from a shared folder. I've sent queries to remove the offending row, but haven't heard anything back from their IT department.

The office is closed today (Sunday), but hopefully, this repository is back online.
