FailedDBInsert issue on logon to repository?

Some of our accounts just started receiving this this am. Some work fine. Any ideas what it might be. I tried logging in using the'bad' accounts from other computers and same issue. What can be done?

Does your Vault server log file give any clues?

Can you look at the server's log file? Does it give any indication of what may be wrong?

You can find the log file on the server in the %windir%\temp\sgvault directory.
