This one is weird and I doubt if you have an answer but I thought I'd tell you about it anyway.
I'm using Vault 4.0.4 with VS.Net 2005 on xp2 and am using the Vault VS.net 2005 client.
I work with one fairly large web application (over 5,000 files). Within this solution is a folder (directly under the root) that has about 250 small image files in it. Periodically Vault will flag all the files in this directory as new files, that is, as pending adds. If I try to commit the operation Vault displays an error message because the files are already in Vault, so my only option is to highlight all the files and select the option to undo the operation. I work with another developer on the same project and he never has this problem.

That is strange. If you turn on logging for the 2005 client and send me the contents after you've seen this happen, that might help.
