Operation Failed on Get Latest Version

I am getting a operation failed when performing a get latest version using the SourceOffsite GUI client v4.1.2.

SourceOffsite Server is setup on a Win 2000 Server machine with Visual SourceSafe v6.0c

Below is the log from the log file.

Server Exception (400): [C:\Program Files\SourceOffSite Server\temp\NItin632727465048906250\ConversionReport.txt] -

OPERATION_FAILED
Server Error: 400

Is every Get operation failing? Are other users experiencing the same problem?

The SOS Server writes files to a temp directory before sending them to the SOS Client. There may be a problem with the file in the temp directory or with the directory itself.

I'd suggest clearing out the C:\Program Files\SourceOffSite Server\temp directory on the SOS Server machine. It's best to do this when users are logged off.
