Error 403 on request fileUploadHandler.ashx Forbidden

When using the External Add URL for adding a new Item to Item Tracking there is an option to add an attachment.
Although this allows the user to perform this action, once the Save button is pressed an error occurs however the Item is successfully created without the attachment.

Any assistance with this would be greatly appreciated.
