Constantly getting "Transaction Failed" message

I am constantly getting a message that says, "An error occurred while trying to end a transaction... Transaction failed. There is no other information provided to help me diagnosis this. It happens repeatedly with a few of my files, but not with the majority of my files.

How can I get more information to diagnose this problem?

What types of transactions are you trying to do in Vault when you get this error? Where do you see it?

What version of Vault are you using?
