sgvault Database Insert Error

We have recently begun receiving the following error when trying to check in files to Vault:

"A database error has occurred(FailDBInsert)"

When I go to the vault log it has the following message.

"Violation of PRIMARY KEY constraint 'pk_tblfsobjectversions'. Cannot insert duplicate key in object 'tblfsobjectversions'"

Can anyone tell me what is going on and how to correct this error?

Can you tell me the version of Vault you are on?

Have you upgraded recently or made any large changes?

Is this happening for everyone?

Is this error occurring in the Vault GUI or in VS with the IDE integration?

We have made no recent changes to Vault or SQL Server.

Yes, it is happening to everyone.

It is happening in the GUI.

Can you tell me what .NET version(s) you have on your server?

Also, can you open your Vault - GUI, connect to a repository, and then go to Help - Technical Support and copy the environment information from there and post it?
