Issue upgrading to Vault v4.0.6 from v3.1.1

When we get to the Admin Password question it does not accept our password. Also, when we try and go to vault after this it is messed up and requires us reinstalling the "old" version to get it to work again.

Any ideas?

I would suggest restoring a good backup of your 3.1.1 database and first upgrading to Vault 3.5.3. Then upgrade to Vault 4.0.6.
